Challenge 1
Challenge 1
"Promote shared management of patients between Oncology and Primary Care"
Objective
We look for solutions that:
- Facilitate communication between Primary Care and Oncology in a way that allows both to manage patients in a more agile and efficient way:
- To Primary Care doctors, allowing them to quickly resolve (on the same day) their doubts about drug interactions, adverse effects and other symptoms of potential relevance.
- For oncologists, providing them with relevant information (patient admissions, etc.) to AP.
- For the healthcare system, allowing them to refer patients to the appropriate service more efficiently.
Context
Less than 5% of breast cancer patients continue to visit their Primary Care (PC) doctor while they are treated in the oncology service. However, in many cases, these patients, who only visit their oncologist, consult him for health problems unrelated to their neoplasia. On the other hand, Primary Care professionals do not always feel comfortable when making decisions about the management of these patients, nor do they find it easy to access the oncologists to solve their doubts.
This situation contributes to the saturation of oncology services and represents an inefficient use of healthcare services. Therefore, we seek to promote better shared management of the care of patients with breast cancer between Primary Care and oncology.
